Appointments are confirmed by the provider. Location, specialty, eligibility, and same-day availability must be checked. Text opens your messaging app; review before sending. Tell us where it hurts, choose a specialty, and request a visit. A participating office confirms the appointment and availability.

Let us help you connect with a legal office to discuss your situation.

A consultation request does not establish an attorney–client relationship. The legal office confirms the consultation and any representation. No case result or financial recovery is promised.
You may represent yourself. A New York workers’ compensation attorney or licensed representative is optional.
You can ask for language help. Use the Board’s official instructions and ask about an interpreter when needed.
Filing is not a promised result. The Board and claim decision-makers apply the official facts, forms, and deadlines.

Full address records and source details are retained privately for verification. The public map does not identify a clinic, provider, or directory brand.

If anyone is hurt, in danger, or needs emergency help, call 911 or local police. Police may investigate, take witness information, and file a separate police crash report.
MV-104 is the driver’s separate report to DMV for a reportable New York crash. It does not replace the police report or a police investigation.

Source checked October 18, 2026. DMV instructions and deadlines control. For urgent medical danger, call 911.
Short examples of how an officer documents a crash and what people usually look for afterward. A report records observations, statements, diagrams, and codes. It does not decide a legal claim or promise a payment.

Officers document location, movement, traffic controls, damage, injuries, statements, witnesses, and available video. The report helps insurers and courts evaluate a claim, but it does not promise a payment or decide every legal issue.
Source: NY DMV P-33 ↗New York cases commonly place the initial burden on the rear driver to give a non-negligent explanation. Photos, stopping distance, traffic flow, sudden lane changes, and witness or dashcam evidence can matter.
Source: New York court example ↗The report may compare lane lines, signal use, mirror or blind-spot issues, damage direction, and each driver’s statement. A lane-change label alone does not answer every fault question.
Source: DMV report-code guide ↗Officers may document the signal, turn arrow, lanes, right-of-way position, speed, and where the vehicles met. Read the diagram and narrative together, and preserve signal, witness, and video evidence.

Let us help you gather the right information and guide you through the C-3. You review and submit the official form yourself.

Where were you, and what were you doing?

Write down who you told and when. WCB says notice should be given as soon as possible and within 30 days.
Supervisor or employer contact you told.
The date you gave notice.
Whether you told them in person, by phone, or in writing.

Provider or hospital name and address.
The date of your first treatment.
Whether the same body part was injured before; WCB may require Form C-3.3.

Keep the first-treatment details with your claim notes.

Use the dates and duties you know. Your provider or employer may keep separate work-status records.
Whether you stopped work and the date.
Whether you returned and when.
Your duties and current employer status.

Use simple dates and plain words. Your provider may give separate restrictions.

Use the official C-3 process, check every answer, submit it to the WCB, and save the confirmation.
Read every answer before you submit.
File as soon as possible; WCB lists a two-year limit for an accident claim.

Choose a symptom infographic, a straightforward call card, an attorney-help card, or the location map. Every design comes in both phone-number versions.
These are creative drafts for review. Text-message capability, office scheduling arrangements, and the public launch destination must be verified before advertising. Paid-ad wording may need platform-specific changes.
The launch package is designed to turn local inquiries into reviewed appointment requests. A request is counted separately from an appointment the practice has confirmed.
| Stage | Action | Completion evidence |
|---|---|---|
| Before traffic | Confirm the legal business name, phone ownership, text delivery, response hours, privacy notice and actual office scheduling arrangements. Choose a public landing destination; this private review site requires sign-in. | A test call/text is received and answered; a test appointment is confirmed by a participating office. |
| Organic distribution | Use approved call-card and location graphics on the business’s social profiles and approved community channels. Link to the verified destination. Obtain permission before posting in groups or distributing through third parties. | Each placement has a source label or campaign link; staff can identify its leads. |
| Paid search draft | Prepare local campaigns around service-intent searches such as “orthopedic appointment near me,” “injury care appointment,” “physical therapy appointment,” and “workers comp doctor appointment.” Choose markets where booking access is confirmed. No budget or campaign has been activated. | Owner approves the account, geographic markets, spending limit, ads, and destination before launch. |
| First traffic review | Review answered calls, qualified requests, contacted offices, confirmed appointments and attendance. Adjust the weakest stage of that sequence. | A daily funnel report distinguishes leads, appointments requested, appointments confirmed, and visits attended. |

Injury Care Appointment Help
Explore local care options after work or auto accidents. Request scheduling help for orthopedics, pain management, physical therapy, chiropractic or neurology. Appointments subject to provider confirmation.
Use service-focused wording rather than asserting knowledge of an individual viewer’s health. Google restricts advertiser-curated audiences for sensitive health advertising. Do not build injury-patient retargeting or upload patient lists. Campaign eligibility and final approval belong to the platform. Google health-advertising policy ↗
| Measure | Definition |
|---|---|
| Answered-call rate | Answered calls divided by inbound calls. |
| Qualified inquiry | A genuine person requesting care or a legal consultation in a serviceable location. |
| Scheduling conversion | Office-confirmed appointments divided by qualified care inquiries. |
| Attendance | Attended visits divided by confirmed appointments. |
| Cost per confirmed appointment | Approved advertising spend divided by confirmed appointments, once spending is authorized. |
This package does not activate ads, spend money, send messages, guarantee patient volume, make medical recommendations, or confirm appointments without an office response. SMS delivery and staff response coverage require live tests. The existing private intake stores requests per signed-in account; a shared production staff queue and public privacy/consent flow need separate implementation before public lead collection.
For New York work-injury workflows, use the saved filing and scheduling reference. Appointment intake does not file a WCB C-3 claim.
